There are a number of forms which must be filled out before your first visit, or if any of the information has changed since your last visit to Dr. Leggett's office

Instructions:

Click on the link below to access the New Patient Form. After you click the link, a browser window will open. After it opens, click on the 'PRINT' icon on your Browser. After the form prints, fill it out and bring them into our office. You can also fax it to our office at 281-580-6693. Completing the New Patient Information Form will save you time on your initial visit!
